Rehab’s Request-a-callback form is closed, and we are not calling anyone back through it. This notice says what happens to any details you gave us there, and how to reach us.
Who is responsible
Rehab is a service of MIRACULOUS EQUATION - UNIPESSOAL LDA (NIPC 518057224), Braga, Portugal, which trades as Ebnia Health and is the controller of any details you gave us on the Request-a-callback form. If you have a question about your data, email us at info@ebnia.com — the address at the foot of every page.
What the form asked for
The form asked only for your name, your phone number and, if you chose, a preferred time of day to call. It never asked for any medical or health information.
What happens to your details
If you gave us your details, email us at info@ebnia.com and we will delete them and tell you when that is done.
Your rights
Under the GDPR you can ask us for a copy of what we hold about you, ask us to correct it, or ask us to delete it. Email us at info@ebnia.com, and we will respond within one month.
If you are not happy with how we have handled your data, you can complain to a data protection supervisory authority — the Portuguese data protection authority, the CNPD (www.cnpd.pt). You can complain to them directly; you do not have to come to us first.
We keep a copy of the exact notice you were shown when you used the form. Email us and we will send it to you.
